Hot or cold. the burning question

Copyright 2006 Robert Ryles Should You Blow Hot or Cold? As a Physical Therapist I am often asked: "Is it better to put heat or cold on an injury?" A really good question in my opinion. It is, however, not surprising that there is uncertainty regarding this. Instant ice packs and chemical cold packs together with hot packs and microwaveable wheat bags are freely available today. All of these come with the manufacturer's claims of pain relief and physical benefits of their use. Being that essentially, on the surface anyway, the two are diamatrically opposite, how can they both be beneficial? If one is good for you is it not likely then that the other is not? Seems sensible. Ok. Let's look at both of these modalities of injury treatment and see if we can throw some light on to this debate. HEAT When heat is applied to the body the following occurs. There is an increase in what is known as local physiology at the site of application. In essence this means that there is a greater amount of bodily activity occurring. Blood flow to the heated area is increased. There is a greater demand for and use of oxygen and nutrients. There are more waste products created. There is a calming or sedative effect on nerve endings in the area. So when and how then is heat useful to us as a tool in the treatment of injury? It is useful to help reduce pain as it has a sedative effect on nerve endings. At the same time it has the effect of increasing local tissue metabolism or physiology. This means it is useful to help stimulate blood flow and bodily activity to promote the healing process by making available the maximum amount of blood, bringing oxygen and nutrients. It also has the effect of being a counter irritant. This means it has the effect of being an effective form of pain relief because the sensation of heat overrides the sensation of pain in the body's nervous system. COLD When ice or cold is applied to the body the following occurs: There is a decrease in the physiology or metabolism in the area of application. The blood vessels tend to shut down to lessen the blood flow to prevent heat loss. The demand for oxygen and nutrients is lessened and there is a decrease in waste production. There is also an associated pain relieving phenomenon. So when and how is cold useful? As it is able to reduce pain, local metabolic activity including blood flow, tissue demands and waste production. it is useful when the desire is to shut down the body's processes. From this analysis of the effects of heat and ice it is obvious that both have a pain relieving capacity. Part from this though the two modalities create totally opposite effects. Heat increases local tissue metabolism and stokes up the fires of activity whilst cold or ice tends to shut everything down. At the point of injury or in the following few days when the condition is still new the aim is to prevent further tissue damage, reduce swelling and shut down as far as possible the activity of the body to allow the injured area to rest before it begins, in earnest, its recovery process. This is when ice or cold therapy is so important. It is only relatively recently that it has been realised how important this is in preventing further problems and even in severe traumas, death. It was discovered during military conflicts that wounded soldiers found, rescued and returned to base wrapped up in blankets and kept warm tended to die where as those initially missed or left out injured in the field to face the cold tended to survive. It was the effect of heat and cold that influenced the outcomes in many cases. The injured soldiers were only able to survive in many cases because the metabolic demands of the body were markedly decreased by the effect of the cold. So cold or ice is fantastic for new injuries to help relieve pain and prevent further swelling and damage. Heat applied at the time of injury will only tend to make matters worse by increasing blood flow, swelling and tissue demands. The story changes somewhat as the injury settles and the healing process begins. After approximately seventy two hours the injured area will normally have settled down and bleeding will have stopped. Heat may be used after this point to relieve pain and stimulate blood flow to enhance the healing process. It is important that it is not applied too soon and relevant expert advice should always be sought regarding this but the seventy two hour rule is a good guide. Heat and ice are both highly effective but can also be dangerous, causing burns and other problems. It is advisable to learn when and how to correctly use these modalities. Keeping the immune system in a fighting mode

They say that, “An apple a day keeps the doctor away.” Good nutrition plays a key role, not only in keeping you from rushing to the medicine cabinet during the season of cold and flu virus, but also in maintaining a healthy immune system. Lacking in one or more of essential nutrients may prevent the immune system from functioning at its peak. Functioning as a complex web of mechanisms inside the human body, the immune system is designed to protect and defend against millions of invaders such as allergens, bacteria, viruses, and other potentially harmful organisms, collectively known as antigens. Serving as the first line of defense against these invaders are the skin and the membranes that line the entrances to the body. If these antigens make it past the skin, they have to face the specialized white blood cells called T-lymphocytes, which continuously patrol the body in search of these body invaders. Other cells that keep the body protected from these harmful germs are B-lymphocytes, neutrophils, and macrophages. B-lymphocytes are special blood proteins that neutralize or destroy germs. Neutrophils and macrophages, on the other hand, scavenge antigens from the blood for delivery to the lymphatic system, which disposes them. It is our obligation to help these cells to function efficiently in keeping our body protected from bacterial infections. While essential nutrients are critical for the production and maintenance of key germ-fighting cells in the immune system, a balanced diet also has a strong effect on vascular function. Note that the immune system is dependent on blood flow because the bloodstream is the route along which infection-fighting cells travel throughout the body to where they are needed. There isn't one specific nutrient or food that can easily vanquish the cold germs and flu virus spells. Maintaining a well-balanced diet and keeping a healthy eating habit are your best assurance to keep the immune system functioning properly. Fruits and vegetables are rich in vitamins A and C. It also has phytochemicals that lend fruits and vegetables their colors. These food groups also contain antioxidants that promote the body's built-in anti-viral and anti-bacterial functions. These nutrients help ensure that the lymphocytes can divide and reproduce properly in response to a virus, and that the neutrophils and macrophages that engulf and kill invading bacteria can do their job. Maximizing the variety of vitamins, minerals, and antioxidants can be achieved by eating 5 to 9 servings of fruits and vegetables per day. It is necessary to consume two different colors of vegetables and fruits with each meal, and one-third of lean protein. How to avoid aging skin

What images come to mind upon hearing the word “old”? Maybe one will think of a person who has white hair, has difficulty walking or hearing, and someone with wrinkly skin. Being old usually has negative connotations and some, if not most, people are even afraid of getting old. This may be the reason why for centuries, people have been looking for that one solution in staying or looking young. During the olden times, people made use of medicinal plants and other natural resources to treat their problems with aging skin. Among these resources are aloe and honey which are still used nowadays. However, with various advances in technology and medicine, more sophisticated treatments have been developed such as injectable fillers, botulinum toxin (also known as botox) and microdermabrasion. These newer solutions may alter or slow down skin aging faster or more efficiently, however, at a higher cost. For those who cannot afford such expensive procedures, there are still other possible skin treatments combating aging that can be done without breaking the bank. Before trying a particular skin treatment, it is important to have even some basic knowledge of the components of our skin and how it functions. As it has been taught in school, the skin is made up of two layers: the dermis which contains various skin appendages, and the epidermis which serves as a protective portion against infections and other threats. Within the skin are also different components and substances such as melanocytes that cause skin pigmentation, collagen that helps form connective tissues, and elastin that is responsible for the skin's elasticity. By knowing the different components and functions of the skin it will also be easier to understand how the skin ages and what can possibly cause it. Causes of skin aging may either be intrinsic or extrinsic. Intrinsic causes may include genetic factors which affect the natural skin aging process. Extrinsic causes on the other hand are those that come from the environment such as exposure to the sun and the environment's humidity level. It has been said that intrinsic factors that cause skin aging cannot be changed or prevented. Often, only the extrinsic factors then are being addressed by most treatments or procedures available. What different treatments or solutions can be done to avoid, prevent or slow down skin aging without much money being spent? 1. Protect your skin from excessive exposure to the sun. If possible, avoid or limit tanning or using tanning devices and products. When doing any activity that may require much sun exposure, make use of sunblocks or sunscreens with a Sun Protection Factor (SPF) of 15 or above. Apply suncreen regularly, 20 to 30 minutes before sun exposure, and take note to re-apply after sweating or washing with water. 2. Moisturize your skin. One indication of aging skin is its being dry. It is important to observe the recommended water intake of 8-10 glasses per day. Also avoid using products that have chemicals which may cause the skin to dry; instead choose those contain moisturizers. Skin moisturizers must be applied after washing. 3. Observe what you eat or drink. According to a medical research director at Revival Soy in North Carolina, soy can help build and maintain collagen which is an important component of connective tissues that make up the skin. A component of soy which is soy isoflavones can also act as antioxidants to protect the collagen from being damaged. It is also advisable to consume food and liquids that are rich in vitamins A, B, C and E which help maintain smooth and youthful looking skin. If desired, food supplements with such vitamins can also be taken. 4. Avoid smoking. It is said that substances in tobacco or cigarettes can cause biochemical changes in the body and skin. Based on a number of researches made previously, smoking can hasten skin aging by increasing the appearance of wrinkles and causing the skin to look yellowish. 5. Avoid repetitive facial expressions and sleeping positions. It is said that constant or repetitive positioning of the face on the pillow may cause lines and wrinkles to be etched on the skin. The same thing happens with repetitive facial expression since when muscles contract to convey these expressions, grooves are formed on the skin. These lines and wrinkles, which are often observed in aging skin, may be more visible as a person grows older and his or her skin decreases elasticity. Treatments other than those mentioned above, specifically those that are not as costly as the ones offered in clinics, can also be used to address skin aging. Among these are use of medicinal plants and natural resources as what the people have done during ancient times. However, it is important to know the substances and reactions of these materials to the skin so as not to be misled and to avoid complications. It is therefore not necessary to spend so much just to avoid or prevent skin aging. Through a disciplined lifestyle and regular observation of the solutions stated above, prevention of premature skin aging can be addressed. Though eventually, a person has to learn to accept that despite of all treatments undertaken, his or her skin will still age naturally. Food is a source of nourishment and energy. Without food we would not be able to perform our daily activities. Our muscles would wither, our nervous system would fail. So, we all need food. However, food also has another role in our lives - a role of comfort and entertainment. Culturally and instinctively we prepare and serve foods to comfort those who have experienced loss, to celebrate joy or to show friendship and love. So what exactly is emotional eating? Seeing food as more than just a source of energy and enjoying it simply for the satisfaction it gives is not wrong. In fact science shows that food can promote good feelings by chemical reactions caused in our brains. What IS a problem is when an individual cannot experience pain, anxiety, joy or even boredom without turning to food as means of dealing with those feelings, or they are obsessed with food, weight and dieting. Emotional eaters turn to food as a source of distraction from dealing with feelings. However, eating these foods leads to feelings of guilt which can only be soothed with more eating, restrictive dieting, excessive exercise or purging. Emotional eaters tend to value themselves based on their weight and how closely they've stuck to their 'ideal' diet. Because of this distorted relationship with food, foods are labeled "GOOD" and "BAD". Emotional eating can also lead to serious eating disorders and depression. - How to tell if you're an emotional eater. Do you turn to food for reasons other than hunger? By making online gambling in the USA a legalized form of adult entertainment it would also bring in an estimated 1.2 billion dollars in taxes to the American government. This money could be used for schools, police and universal healthcare for all Americans. This is how you improve society, not by telling people what to do. It is human nature to want what we can not have so the more laws you put in place to stop people from doing something the better the chances are that people are going to do it. People are just as likely to become addicted to gambling at a regulated casino as an unregulated one, but the difference is that in a regulated casino they will not extend you the amount of credit that will get you into trouble in an illegal casino. And in a regulated casino they will have information on how to get help if you have a gambling problem. In an illegal casino they will not have this type of information, they want you to continue to gamble till you have nothing left, and then they will let you fall and find someone to take your place. In the long run society can only prosper if we educate people on the dangers of gambling both online and in a casino, and not from banning an industry that employs thousands of people in countries all over the world. No one gains anything from just telling people they can not do something that they are going to do anyway. It is about time the government learned this and stopped making the same mistakes year after year.

Problems in the Achilles tendon are common in dancers and it is a generally poorly understood injury in many dance circles. It is important that several other issues that may give you pain in the back of the ankle are ruled out first, before trying this program, so check with a sports doctor or physiotherapist / physical therapist before you go ahead. Other things that may give you pain behind the ankle include may an extra bone at the back of the ankle called an 'os Trigonum' and a syndrome called 'Posterior Impingement' If you do have a problem with the Achilles Tendon, it responds best if treatment is started at an early stage, but even then it may take a while to heal (sometimes in excess of three months) Long-standing Achilles tendonopathy may require an intense rehab program of up to six months. This is because it has usually developed over a long period of time, and is usually due to weaknesses or imbalances in several different areas, so it is important to correct all of these to gain a full recovery. The keys to successful rehabilitation of Achilles tendonopathy are; Early diagnosis and RICE treatment to prevent further damage An Eccentric Strengthening Program (described below) Correction of any predisposing factors such as muscle tightness, weakness or abnormal movement patterns Core Strengthening Program Appropriate progression to functional activities and sport-specific rehabilitation. If you see a therapist, they may do some massage and mobilization techniques to make sure that all the bones in your feet are moving properly. This is very important, as stiffness in the foot may have been one of the major initial causes of the injury. Eccentric exercises" are exercises where the muscle is contracting while it is lengthening. This sounds odd, but is possible! While these exercises have been shown in several studies to be the best thing for rehabilitating Achilles problems, they do have the potential to cause damage if performed inappropriately or excessively. You should always start with an adequate warm-up and commence the exercise cautiously. It is normal, however, to get some pain when you begin the strengthening program and at each new load. You should only progress to the next exercise when the previous version is pain-free both during and following the activity, and on the advice of a qualified medical professional. Always ice the area (on a stretch) after doing these exercises. Eccentric Exercises. Bilateral heel drop. Place feet on to the edge of a step in a demi-pointe position (use a barre for support). Slowly, lower your heels below the level of the step. Repeat for three sets of 15, twice a day. Make sure that your feet and arches stay in a good position the whole time! Arches on! Remember not to rise up from the low position. but place your feet back into the starting position. Contracting the calves via rises may aggravate the injury. Do some sets with the knees straight, and some sets with the knees bent to target the deep calf muscles more (Soleus) Increase the amount of weight through the injured leg. Single leg heel drop. Rise onto demi pointe on the edge of the step with both feet. Transfer the weight to one foot, and slowly lower the heel to below the edge of the step. Add weights NB: Ice should always be applied following exercise. As with any overuse injury, it is important that the predisposing factors are corrected. Things that any supervising medical professional should be looking at include: Generalized tightness of the calf muscles Reduced range of motion in the ankle or mid foot joints Control of arch and small toe muscles (see: The Perfect Pointe Book!) Pelvic Stability Abdominal control Return to activity should be gradual. You may usually return to dancing when any tenderness to touch has settled. Until then it is best to do a 'Floor Barre' to avoid extra loading on the tendons, but to maintain mobility and strength. Doing other exercises such as Pilates will help speed your return to full dancing and stop you from getting too weak! On return to activity, a small heel raise may be used to reduce the load on the Achilles tendon (in both shoes).

Bone hormone a startling discovery for diabetes treatment

A startling new discovery about a hormone released from the bone is significantly changing scientists’ understanding of diabetes and giving new clues about how to deal with the “Big D.” Considered to be the fifth leading killer of Americans, diabetes is a disease in which the body’s failure to regulate blood sugar (glucose) can lead to serious and even fatal complications. The regulation of glucose entails the body's monitoring of how much sugar is present in a person’s blood; how much is taken up by cells for fuel; and how much is released from energy stores. These processes are performed by the pancreas, the liver, muscles, and fat. Other specific types of diabetes, which may account for 1% to 2% of all diagnosed cases, result from specific genetic syndromes, surgery, drugs, malnutrition, infections, and other illnesses. However, new research suggests that the issue is even more complex than what it seems to be. A hormone from the skeleton may influence how the body handles sugar. There is also an increasing evidence that demonstrates that the signals from the immune system, the brain and the gut play very important roles in controlling glucose and lipid metabolism. These findings are mainly relevant to Type 2 diabetes, the more common kind, which comes during adulthood. While it is true that having elevated blood sugar is the defining feature of diabetes, the reasons for abnormal sugar tend to be different from one individual to another. It is in understanding exactly what signals are involved that raises the hope of providing the right care for each person each day, rather than giving everyone the same drug. When researchers from Columbia University Medical Center published the results last summer, scientists were astounded that a hormone released from the bone may help regulate blood glucose. Lead researcher, Dr. Gerard Karsenty, first described the findings at a conference where the assembled scientists appeared to be overwhelmed by the potential implications of the study. It was the first time that the skeleton was actually seen as an endocrine organ, producing hormones that act outside of bone. In his previous work, he had shown that a hormone produced by fat, called leptin, is an important regulator of bone metabolism. In this work, he tested the idea that if fat regulates bone, bone in essence must regulate fat. His experiment with mice revealed that a previously known substance called osteocalcin, which is produced by bone, acted by sending signals to the fat cells as well as the pancreas. The net effect is to improve how mice secrete and handle insulin, the hormone that helps the body move glucose from the bloodstream into cells of the muscle and liver, where it can be used for energy or stored for future use. Insulin is also important in regulating lipids. Patients with Type 2 diabetes no longer heed the hormone’s directives due to the cells' resistance to insulin. Their blood glucose levels surge and production of insulin in the pancreas declines as well. The experiment revealed an increase in osteocalcin which addressed the twin problems of insulin resistance and low insulin production. The mice became more sensitive to insulin and it increased their insulin production, thus bringing their blood sugar down. As a bonus, it also made obese mice less fat. Should osteocalcin works in humans as well, it can be considered as a “unique new treatment” for Type 2 diabetes. Most current diabetes drugs either raise insulin production or improve insulin sensitivity, but not both. Drugs that increase production tend to make insulin resistance worse. A deficiency in osteocalcin could also turn out to be a cause of Type 2 diabetes. The immune system is considered to be another cause of glucose regulation. In 2003, researchers from two laboratories found that fat tissue from obese mice contained an abnormally large number of macrophages, immune cells that contribute to inflammation. Scientists have long suspected that inflammation was somehow related to insulin resistance, which precedes nearly all cases of Type 2 diabetes. In the early 1900s, diabetics were sometimes given high doses of aspirin, which is an anti-inflammatory. Only in the past few years has research into the relationship of obesity, inflammation and insulin resistance become a serious concern. A number of researchers agree that obesity is accompanied by a state of chronic, low-grade inflammation in which some immune cells are activated, which may be a primary cause of insulin resistance. They also agree that the main type of cell responsible for the inflammation is the macrophage. Should more research prove the initial findings to be true, there would be certainly greater hope of relief and treatment for diabetics everywhere.
